The TX Health Benefits Pool made the decision earlier this week to terminate their entire books for ESD, Fire Department, EMS, Police and Sheriff Departments effective 1/1/2024.

- The THBP claims request form letter. This form will tell THBP to send your claims data to Tom. You'll need to insert your Department name and print on your own letterhead.
- A census of your member and enrolled dependent information.
- A copy of your current THBP plan and rates.
